Requests for information or records held by the Office may be made by letter or application form. The application forms are obtainable in our Office at 12/F, 248 Queen's Road East, Wanchai, Hong Kong. The application should be addressed to: Access to Information

| 4. | Our Office may refuse to disclose information, or may refuse to confirm or deny the existence of information, in the categories and for the reasons set out in http://www.access.gov.hk/code.htm. | ||||||||
| 5. | Other than publications which are available free of charge, or at cost specified, photocopying charge (in black and white on A4 size paper) of HK$1.5 per page will be levied. The charge may be subject to revision. | ||||||||
